It's is going to be hard, because this is what you have to do:
Complete Surgery when patient's lifeline is under 5. Usually achievable in Surgery #4.
And with the hacked version their health is 999999.

Yep...pretty much so. You'll need a good trainer if you realy want it.

No,DWD i think what your saying is Near-Death on Dark Cut 3.(I'm not sure though...)
I'm not sure, but i think you gotta finish the surgery on minimum drunkness.(I think...)
